CA SUMMER EXPERIENCE
The CA Experience provides students with opportunities to take part in teacher-led classes/activities in the summer. Classes include but are not limited to fine arts, athletics, academics, etc. Each class varies in the length of time it is offered and in the cost of the program.

This Summer Camp Participation Agreement, Waiver, and Release of Liability (“Agreement”) is entered into by and between Clarksville Academy (“Academy”) and the undersigned parent/guardian (“Parent/Guardian”) on behalf of the participating minor (“Participant”).

1. Acknowledgment of Risk
I understand that participation in summer camp activities, including but not limited to sports, outdoor recreation, field trips, use of equipment, and group activities, involves inherent risks. These risks may include, but are not limited to, bodily injury, illness, permanent disability, or death.
I voluntarily assume all risks, both known and unknown, associated with my child’s participation in these activities.

2. Medical Authorization
I certify that my child is physically able to participate in camp activities. I authorize the Academy and its staff to obtain emergency medical treatment for my child if necessary. I agree to be responsible for any and all medical expenses incurred.

3. Release and Waiver of Liability
To the fullest extent permitted by law, I, on behalf of myself and my child, hereby release, waive, and discharge Clarksville Academy, its trustees, directors, officers, employees, volunteers, agents, and representatives from any and all claims, liabilities, damages, or causes of action arising out of or related to participation in the summer camp program.

This release includes claims based on negligence but excludes gross negligence or willful misconduct where prohibited by law.

4. Indemnification
I agree to indemnify and hold harmless Clarksville Academy from any claims, demands, or actions brought by or on behalf of my child or any third party arising from my child’s participation in camp activities.

5. Code of Conduct
I understand that participants are expected to follow all camp rules and staff instructions. The Academy reserves the right to dismiss any participant for inappropriate behavior without refund.

6. Photo and Media Release
I grant permission for Clarksville Academy to use photographs, video recordings, or other media of my child for promotional or educational purposes without compensation.

7. Transportation
I acknowledge that transportation may be provided for certain activities and consent to my child’s participation. I understand that transportation also carries inherent risks.

8. Severability
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

9. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of Tennessee.

10. Acknowledgment and Signature
I have carefully read this Agreement, fully understand its terms, and sign it voluntarily. I understand that I am giving up substantial legal rights on behalf of myself and my child.
