H1N1 Clinic Wednesday, December 9

On Wednesday, December 9, 2009, we are giving an H1N1 clinic on campus in the Dorothy Martin Library from 8:00 am to 10:00 am. This clinic is offered to any student, family member, faculty or staff member, that would like to take advantage of this opportunity. It is one more step in our process to remain pro-active in keeping healthy in this active flu season.
There will be both nasal spray and injections available at this clinic. The cost for each person is $15. Please bring cash or check payable to Maxim Healthcare Services. They also accept Medicare, Aetna insurance and United Healthcare insurance. Please bring your insurance card to the clinic.
Each person receiving the vaccine will need to have a consent form signed. The forms are available at Mrs. Turnage’s desk or in the nurses’ office. Please use the sign up sheet located in the main foyer or the teacher’s work room, so we can be prepared with enough vaccines.
